Field Permits must be submitted to Kevin Ryder at 124 North Street, Medfield, MA 02052. In accordance with the process currently in place with other town departments, Medfield Parks & Recreation Dept will forward a copy of your department’s application form along with any information used to advertise the program to be offered to the Board of Health for review. Upon review, the Board of Health Agent will make a determination as to whether or not a camp permit is required and the applicant will be notified in writing. It is necessary to allow at least 90 days for the Board of Health to review a Camp Permit and a determination must be made every year.
Pfaff Community Center Rental Form
Please fill out a Pfaff Center Rental Form and return to the Pfaff Community Center, Parks & Recreation Office, 124 North Street, Medfield, MA 02052. Forms should be submitted with 2 checks: 1 for the rental fee and 1 for a cleaning and key deposit (the deposit check will be shredded or returned). Rental Forms will be reviewed and approved by the Parks & Recreation Commission twice a month. Please plan accordingly. The building may be rented Monday through Friday after 6:00PM for meetings. The building may be rented Friday night through Sunday night for parties. The building may not be rented Monday through Friday before 6:00PM because it is being used for Parks & Recreation Activities and the Medfield After School Program.
